Vital Bridges helps people throughout metropolitan Chicago impacted by HIV and AIDS to improve their health and build self-sufficiency by providing food, nutrition, housing, case management and educational services.

From six sites, Vital Bridges serves more than 2,400 people living with HIV and AIDS in metropolitan Chicago. Four of our sites are located in the city, serving neighborhoods such as Edgewater, Garfield Park, and Englewood. The remaining two sites are in the suburbs of Elk Grove Village and Oak Park.

Most of our clients fall well below the poverty line with an average income of less than $600 per month. Ranging in age from under 2 to 70, our clients include men, women and children from diverse racial and ethnic backgrounds.

Food Services
Vital Bridges provides more than 530,000 culturally appropriate, nutritious meals annually through grocery distribution and delivery of prepared meals.

Housing Program
Each year, individuals and families receive more than 50,000 nights of housing and help in building their skills to secure and maintain permanent independent housing.

Case Management
Case managers partner with clients to assess immediate and long-term needs, develop a plan to respond and link clients to needed services. Vital Bridges’ case managers conduct 14,000 client sessions each year.

Nutrition Program
Annually, Registered Dietitians conduct 1,000 one-on-one counseling sessions with clients to help them develop balanced diets and manage their mediation regimens to improve and sustain their health.

Educational Services
Through tutoring, computer literacy training, life skills workshops, and job readiness training Vital Bridges helps clients improve their basic education and vocational skills. About 2,400 sessions are conducted on an annual basis at our learning center.
