In July 2011, Vital Bridges joined with Heartland Health Outreach, a subsidiary of Heartland Alliance for Human Needs & Human Rights, to become the Vital Bridges Center on Chronic Care. This partnership ensures that we can continue to provide the quality food, nutrition, housing and case management services our clients depend on, while allowing us to expand to include access to Heartland Health Outreach's outstanding medical services.

When you donate to the Vital Bridges Center on Chronic Care, you make possible bags of nutritious food and warm home delivered meals, nights of safe and affordable housing, hours of critical counseling sessions and numerous educational and vocational training classes for men, women and children living in poverty with HIV and AIDS. Our name has changed and the scope of our operations is expanding, but the impact of your gift in the lives of the people we serve remains as important as ever.

Giving Options

In addition to making a one-time gift to the Vital Bridges Center on Chronic Care, you can also become part of our monthly giving program, Extra Helpings, or make a gift in memory or honor of a loved one. As our valued donor, we want to make your gift giving convenient and accessible to you. Please consider making a contribution today.

Support Vital Bridges through a Recurring Donation

Consider our monthly pledge program - Extra Helpings. You decide on the level of your gift and we charge your credit card on the first of every month. This may allow you to give more generously than you originally considered. It adds up – a $25 monthly gift equals $300 a year! At the end of the year we’ll prepare and send you a statement, including a record of all your gifts for your income tax records.
